Measles Exposure in Pregnancy

Assessment and Investigation

Step 1 – assess immunity by clinical history Immunity is assumed without further testing if ANY of the following:

  • Documented 2 doses of measles-containing vaccine (MMR or MMRV vaccine), or
  • The contact was born before 1970 in the UK (assumed to have natural immunity from past exposure)
Step 2 – assess immunity by blood test If immunity cannot be assessed by history (e.g. patient is uncertain) → perform serum IgG testing

  • +ve IgG = immune
  • -ve IgG = non-immune

Management – Post-Exposure Prophylaxis (PEP)

PEP is only indicated for non-immune pregnant women.

There are 2 main components of PEP:

  • Immunoglobulin
    • Immunocompetent → HNIG
    • Immunosuppressed → IVIG
  • MMR vaccine – to be given after delivery
