Choice of Surgical Technique

Intracapsular Fracture

  • Displaced intracapsular fracture → hip replacement (arthroplasty) with cemented implants
    • Total hip replacement is preferred for fit patients
    • Partial hip replacement for unfit patients

 

  • Undisplaced intracapsular fracture → internal fixation (cannulated screws or sliding hip screw)
    • This is not explicitly stated by NICE, but is standard practice

Extracapsular Fracture

  • Subtrochanteric fracture → intramedullary nail
  • Intertrochanteric fracture → sliding hip screw
